A simple Enduring Guardian is right for you if only want to appoint one person as your guardian.
In the event you lose mental capacity through an accident, or medical condition, your guardian will be able to make the following decisions for you :
Where you live;
What services you receive;
What healthcare you receive;
To give consent to medical and dental treatment; and
Request personal information about you from your service providers, such as your test results etc.
You will not receive any legal advice with our Online Simple Enduring Guardian.
If you require legal advice, or you wish to appoint more than one guardian, or you want to limit the decisions your guardian can make, you will need our Standard Enduring Guardian.

Complete and submit the online form. It should take you around 10 minutes.
You receive the locked PDF draft of your Simple Enduring Power of Attorney in your email inbox (for you to review) within 2 business days along with our tax invoice.
You pay the tax invoice and within 1 business day you will receive the unlocked/ printable PDF of your Simple Enduring Power of Attorney along with instructions on how to have the document validly executed in your email inbox.
You then attend your local court house to have the document properly executed (an appointment will be required). If you cannot attend a local court house, we can arrange for you to execute the document with us via video link but this incur a further charge of $50).
